The documents you need depend on your business structure. In every case you’ll need an MHV Membership Application, an MHV Business Resolution of Authority Card, and a NYS Driver’s License / Photo ID with a street address (if a PO Box, proof of residency is required). Additional documents by structure:
Sole Proprietor / DBA: Copy of DBA / Sole Proprietor Business Certificate filed with the County Clerk.
Partnership: Certification of Beneficial Owners; Partnership Agreement Business Certificate filed with the County Clerk.
Association / Organization: Certification of Beneficial Owners; letter from officers authorizing the account and stating authorized signers.
Corporation / Sub S: Certification of Beneficial Owners; copy of Corporate Filing Receipt.
LLC / LLP: Certification of Beneficial Owners; Articles of Organization; copy of LLC/LLP Filing Receipt.
